Community Health Workers for Medi-Cal Members in the Perinatal Period
'TLC' is one on one care coordination, health education, and non-clinical support for pre- or intraconception, pregnant, or postpartum Medi-Cal members. Our PCHWs are trained in perinatal mental health, community resources, health navigation, childbirth education, breastfeeding support, and postpartum planning. TLC is a touchpoint between prenatal appointments, hospital discharge, and postpartum follow-ups. *Free diapers and wipes for all TLC members
